Instructions:

1. FILL IN THE FORM: Fill in the blanks below and check the appropriate boxes. DO NOT PRESS THE RETURN key to advance to the next blank because doing so will automatically submit your information before you have a chance to complete the form.
2. SUBMIT: Click the submit button and your information request form will automatically be emailed to Laurie Jodice in the TCS 17 office at Oregon State University.
3. PRINT: If you want a record of you submission, print the "Thank You " page after you click "Submit"

The Coastal Society encourages the use of email in providing the above information to the Conference Committee.
